Chart Notes of an Adult ADD Practice

The chart notes in this section are intended to provide a picture of how I carry out my assessment and treatment of patients referred to me with possible Adult ADD.  The clinical materials are based on actual cases I have seen.  Names, dates, and details have been changed to protect patients' confidentiality.  The notes have also been edited to include only the relevant highlights of my contacts with these patients.

Maria J. (called for help after she took her daughter's Ritalin and found it helped.)

Neil D.  (found that Adderall made him too rigid, lost his sense of humor; lowering the dose eliminated this problem and controlled his ADD symptoms.)
